Our Office Policies

We have your convenience in mind. With the NEW online patient registration portal you can fill out your patient registration any time of day. Registering online will save time when you arrive for your appointment. This will ensure that you are seen as quickly as possible when you arrive for your appointment. Registration only takes about 10 minutes. Before you start the registration process, have your medical and vision insurance cards at hand. Fill out the information as accurately and completely as possible. You must fill out all required fields for the registration process to be complete. Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

To schedule an appointment please call us at 813-852-0012. We understand that your schedule may change and that emergencies arise. If you are unable to keep your appointment, we ask that you provide us with 24-hour notice. Please extend us this courtesy so that other patients may be seen.

Patients who miss their scheduled appointment and do not contact our office ahead of time or immediately thereafter with a valid reason (i.e. hospital visit or illness), a $50.00 no-show fee will be applied to your account and will be collected before any future visits.

Emergencies

Our office provides on-call service for emergencies. Call 813-852-0012 so that the physician on call can be contacted.

We provide 24-hour emergency coverage for all patients. If you have an emergency after office hours, call our office at 813-852-0012 or our answering service at 813-852-0012. The answering service will take your message and contact a covering physician immediately.

Payment

The Tampa Rheumatology accepts cash and personal checks as well as the following credit cards:

  • Self Payment Option Available Upon Request

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.

The Tampa Rheumatology provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
